Eventide on Eighth - Transitional Care Unit

At Eventide's Transitional Care Unit, we share your goal of returning home as soon as possible after illness or injury. However, sometimes a hospital stay is not long enough to provide optimal recovery. Our Transitional Care Unit is designed to bridge the gap between hospital and home, offering rehabilitation and sub-acute care services. Our facility offers and independent short-term rehab unit, fully equipped to provide all the comforts of home.

Your physical, occupational, and speech therapists will create a comprehensive therapy plan, designed for your optimal treatment and recovery. Progress is evaluated continuously, while mobility skills and activities are practiced to restore freedom of movement, self-confidence and functional independence.

As you prepare for your return home, we offer the support and services needed, including a home visit and evaluation if required, to help ensure a safe transition.
