Transitional Care

At Eventide’s Transitional Care Units, we share your goal of returning home as soon as possible after illness or injury. However, sometimes a hospital stay is not long enough to provide optimal recovery. Our Transitional Care Units are designed to bridge the gap between hospital and home, offering rehabilitation and sub-acute care services. Our facilities offer independents short-term rehab units, fully equipped to provide all the comforts of home.

Our in-house team of professional therapy staff enables you to build a close relationship with your therapists, ensuring continuity of care. Your physical, occupational, and speech therapists will create a comprehensive therapy plan, designed for your optimal treatment and recovery.

Progress is evaluated continuously, while mobility skills and activities are practiced to restore freedom of movement, self-confidence and functional independence.

As you prepare for your return home, we offer the support and services needed, including a home visit and evaluation if required, to help ensure a safe transition.

Eventide’s Transitional Care Units offer the following:

24-hour skilled nursing care
Individual care plans and regular assessments
A consistent team of physical, occupational and speech therapists
Beautifully decorated private and semi-private rooms with flat screen televisions and telephones
Delicious meals served in private dining rooms
Social areas for visiting and activities
Admissions accepted seven days a week
Out-patient therapy Monday through Friday
